Physical failures
Ticking, scratching, or clicking noises usually mean the read/write heads, motor, or platters have failed. Physical damage can also come from mishandling or circuit-board failure. These almost always cause at least partial data loss, but the right tools and a clean environment can still recover a lot.
Logical failures

Secure disposal
"Delete" doesn't actually destroy data. Almost nothing about it does. We properly dispose of magnetic and optical media so retired drives, tapes, and discs can't be recovered by anyone else. The right method depends on the media; we'll choose it for you.
